These terms apply when you book a treatment with Phenix Med Spa through our online booking page.

Booking an appointment

• Submitting the form requests an appointment at the time you selected. We confirm it by text, email or phone.
• Please arrive a few minutes early. If you are more than 15 minutes late we may need to shorten or reschedule your treatment.
• To change or cancel, contact us at +1 437-476-1899 or booking@phenixbeauty.ca. Please give us at least 24 hours' notice where you can.

Treatments and results

Our treatments are cosmetic skincare services. Results vary from person to person and we make no guarantee of a specific outcome. Our information is not medical advice. Tell your provider about any allergy, skin condition, medication or pregnancy before your treatment, and speak with your doctor if you are unsure whether a treatment suits you.

Promotional gifts and codes

• Gifts and add-ons are given at your visit, one per guest, while supplies last.
• They have no cash value, cannot be exchanged for cash, and cannot be combined with other promotions unless we say otherwise.
• A code applies only to the appointment it was issued for.

Text messages

Appointment and offer text messages are sent as described in our Privacy Policy. Message frequency varies and message and data rates may apply. Reply STOP to stop messages or HELP for help. Consenting to messages is not a condition of booking.

Carrier liability

Mobile carriers are not liable for delayed or undelivered messages. Delivery depends on your carrier, your device and your coverage, and we cannot guarantee that every message arrives. If you are expecting a confirmation and do not receive one, please call us at +1 437-476-1899.

Age restriction

Our treatments and our text messaging program are for adults. You must be at least 18 years old to book an appointment and to consent to receive text messages from us. By booking or opting in you confirm that you are 18 or older. If you are under 18, a parent or legal guardian must book and attend with you.

Changes
